SAMBA ID Cards - Standard Option

Please Note

  • All members receiving SAMBA/CIGNA medical ID cards will receive one card for each family member. Each family member will receive an envelope with a personalized ID card. (If you don´t receive a card for everyone in your family, or if you need a replacement card at any time, contact SAMBA using the Request Form below or by calling 800-638-6589.)
  • New members will receive a SAMBA/Medco prescription drug ID card. Each household will receive a packet containing two ID cards; both cards will have the member´s name. If you were previously enrolled with SAMBA, you will not receive a new SAMBA/Medco card. You may request additional cards as needed; please contact SAMBA as explained above.

 

1. If you have self only coverage and do not have Medicare A & B primary coverage, or if you have family coverage and not everyone in your family has Medicare A & B primary coverage, these are your ID cards:

 

MEDICAL CARD

This card displays the CIGNA logo so that providers of service will know that you have access to the CIGNA PPO Network. Give this card to your medical providers at the time of service.

 

PRESCRIPTION DRUG CARD

This card displays the logo for Medco, our prescription drug benefit program manager. Present this card to the pharmacist when you need to fill a prescription.

 

COMBINATION MEDICAL AND PRESCRIPTION DRUG CARD

2. If you have self only coverage and have Medicare A & B primary coverage, or if you have family coverage and everyone in your family has Medicare A & B primary coverage, this is your ID card:

This is a combination medical and prescription drug card. Use it for medical services and when you are purchasing prescription drugs.
