If you live in Delaware, the District of Columbia, Maryland, or Virginia:

Use a CareFirst Blue Cross Blue Shield dental claim form and follow the instructions on the form. Claims must be filed by December 31st of the year after the year you received services. You are responsible for making sure your claims are filed within the timely filing deadline.

Standard Option dental claims are to be sent to the following address for processing:
