Q. Who is eligible to enroll in the Dependent Children Health Benefit Plan?
A. Only members enrolled in the SAMBA Health Benefit Plan may enroll their eligible children ages 22 through age 26 in the plan. An eligible child means a members recognized: 1) legitimate child; 2) an adopted child; a stepchild, foster child, or recognized natural child who lives with the member in a regular parent-child relationship; 4) a recognized natural child for whom a judicial determination of support has been obtained; or 5) a recognized natural child to whose support the member makes regular and substantial contributions.
Q. Does my child have to be a full-time student to be eligible to enroll?
A. No, the plan is open to students and non-students.
Q. What type of coverage is included with the plan?
A. The Dependent Children Health Benefit Plan benefits are the same as the benefits included in the SAMBA Standard Option Health Benefit Plan. Not only does this plan cover medical expenses, it also includes routine dental care.
Q. How much does the coverage cost?
A. The premium is $120.00 biweekly or $260.00 monthly for each child enrolled.
Q. When can I enroll my child?
A. You must enroll your child: 1) within 60 days of their 22nd birthday; 2) within 60 days of your enrollment in the SAMBA Health Benefit Plan; 3) within 60 days of the childs loss of other health insurance coverage; or 4) during the regular open season period held for the FEHB programs, normally held the 2nd week of November through the 2nd week of December each year. When enrolling during open season, coverage would not be effective until the following January.
Q. How do I enroll?
A. Download an enrollment form from our website, or call us at 1-800-638-6589 and we will send you a form. Fill out the form and mail it to SAMBA Group Plans, 11301 Old Georgetown Rd, Rockville, MD 20852.
Q. What happens if a child enrolled in the Dependent Children Health Benefit Plan gets a job?
A. If your child obtains a job that offers health insurance, he or she is no longer wholly dependent on you and is no longer eligible for the DHBP. You should notify us of this change by calling SAMBA at 800-638-6589.
Q. What happens if my child gets married while in the Dependent Children Health Benefit Plan?
A. Married children are not eligible for coverage in the DHBP and their coverage will terminate on the date of marriage.
Q. What happens if my child enlists in the military?
A. Children who enlist in military service are not eligible for coverage in the Dependent Children Health Benefit Plan.
Q. Is my child covered anywhere in the country?
A. Yes. Just like SAMBA Federal Health Plan members, Dependent Children Health Benefit Plan enrollees are covered wherever they go, including overseas.
Q. Can he/she get dental coverage?
A. Yes. Children from age 22 up through age 26 can enroll in SAMBA´s Dental and Vision Care Plan, if their parents are already enrolled.

Q. Does the coverage end on my childs 27th birthday?
A. Coverage will terminate on your childs 27th birthday, provided they are still eligible for coverage at that time.
Q. When my child turns age 27, is there any other health insurance available?
A. Yes. SAMBA will send you information about conversion plan coverage if you are interested.
Q. How do I cancel my Dependent Children Health Benefit Plan coverage?
A. You may voluntarily cancel your coverage at any time. To do so, send your written request to SAMBA at 11301 Old Georgetown Rd, Rockville, MD 20852, by fax to 301-816-0191, or by email to healthplan@sambaplans.com.
