HOMELINK's patient placement process is designed to make your life easier; you make one phone call to 1-877-726-2204, and our Patient Care Coordinators do the rest.  There is never a charge for referral sources using HOMELINK services.

Here's how HOMELINK works for you:

  • Call HOMELINK at 1-877-726-2204 and provide your group number for quicker response
  • Give your HOMELINK staff member the necessary patient and medical information along with the specific equipment/services needed
  • Contact the dealer to arrange for services needed by the patient
  • Contact the referral source with expected delivery time and confirmed price
  • Follow up with the patient to assure the vendor provided quality service

HOMELINK will bill SAMBA for all equipment and services based on our contracted rates.  We will then wait to receive a copy of your Explanation of Benefits and if there is a balance to be paid, HOMELINK will send the enrollee a detailed bill explaining the charges
