SAMBA offers Health Care Providers a more efficient way to check eligibility, view claims status, and receive payment and remittance advices.
Online Eligibility and Claim Status
Due to the Change Healthcare cyberattack, these services are currently unavailable. SAMBA is working to restore these functions as quickly as possible.

Electronic Funds Transfer
To eliminate paper checks and receive Electronic Funds Transfer (EFT) payments, please complete the SAMBA EFT Authorization Agreement Form.
Electronic Remittance Advices
To receive Electronic Remittance Advices (ERAs), please sign up for an account on the Availity website and then enroll to receive ERAs from SAMBA.
If you need additional assistance after you login to Availity, please access the training tutorial on receiving ERAs or contact Availity Client Services at (800) 282-4548, Monday-Friday, 8:00 a.m. to 8:00 p.m.
Precertification
The SAMBA Health Benefit Plan requires precertification for a number of different services including inpatient hospital stays. Please visit the Precertification Requirements page for more information.
Coordination of Care
To help facilitate communication between providers, Cigna has developed communication tools which are available to download. These tools may serve as a model for exchanging clinical information throughout the coordination of care. Please visit Cigna’s Continuity and Coordination of Care page for more information.
Independent Dispute Resolution (IDR)
If you are a non-participating provider interested in disputing payment for certain services in accordance with the No Surprises Act, please contact Ermer & Suter to learn more about the IDR process.
Overpayment Recovery
If you are a provider that receives an overpayment notice from SAMBA, please contact Third Party Recovery Services to learn more about the recovery process.
