IRS Form 1095-B
Form 1095-B provides information abut the minimum essential health coverage you had during the year.
Under IRS rules, SAMBA is not required to automatically mail Form 1095-B to individuals because information from Form 1095-B is no loner required to file a federal income tax return. Beginning with the 2019 tax year, the IRS penalty for not having minimum essential health coverage was reduced to zero. However, we recognize that some states maintain a requirement for reporting health coverage so SAMBA will provide Form 1095-B to members upon request.
