IRS Form 1095-B
Beginning with the 2019 tax year, the IRS penalty for not having health coverage was reduced to zero. As a result, information from Form 1095-B is no longer required to file a federal tax reform.
We recognize that some states maintain a requirement for reporting health coverage so SAMBA will provide Form 1095-B to members upon request.
To receive your copy of Form 1095-B, a written request must be submitted by:
